ADMISSION REQUIREMENTS - COLLEGE 

Requirements for Admission Processing and Test Scheduling

• Accomplished College Admission Application Form
• 4 pcs. 2x2 pictures (to be used for the forms)
• Copy of Transcript of Records (for Transferees)
• Accomplished Student Information Sheet
• Photocopy of High School Report Card
• Photocopy of Birth Certificate (for HS graduating students)
• Original copies of documents are to be presented during application for verification purposes.

Requirements for Registration / Enrolment

• Form-137 (Secondary Permanent Record for HS Gradutes)
• Certificate of Good Moral Character
• Certificate of Honorable Dismissal or Certificate of Transfer Credential (for Transferees)
• Medical Certificate
• Original Form-138 (Report Card for HS Graduates)

Additional Requirements for Foreign Applicants

• ACR (Photocopy) / SSP
• Certification of Grades/Transcript of Record (for Transferees)
• Passport / Visa (Photocopy)
• Accomplished Recommendation Form
• Parents' Pictures
